Grampians Health Edenhope HUB recognised for holistic delivery
Imagine if you could get a community nurse to sit down with your physiotherapist, mental health practitioner, dietician, occupational therapist and social worker, to figure out a holistic approach to your health issues.
In Edenhope, at the Grampians Heath HUB, that is a regular occurrence.
“With clients’ permission, we often gather as a team and discuss the best way forward for them,” Primary Health Coordinator Jo Grant said.
“Our district nurses work side-by-side with allied health assistants and when they see a need, whether it’s an inability to complete daily activities or signs of depression, they are right next door to a professional that can help.”
The innovative approach of the HUB team has been recognised – it is a finalist in the Victorian Rural Health Awards, for ‘outstanding contribution by a rural multidiscipline health team’.
The one-stop-shop approach was introduced four years ago, transforming the delivery of services in Edenhope.
“It is difficult to articulate the effect of bringing so many passionate people together, in one place, to meet community needs,” Ms Grant said.
“The design of the HUB is innovative and transformative, but its true success comes down to those individuals going above and beyond every day.
“We’re a tiny team making a huge difference and I’m so glad that’s being recognised.”
Victorian Rural Health Award winners will be announced on June 20.
